HCBA Waiver Agency
After a competitive bidding process, Home Health Care Management, Inc. was awarded a contract by the Department of Health Care Services (DHCS) to provide administrative services and Comprehensive Care Management services, under the Medi-Cal Home and Community Based Alternatives (HCBA) Waiver Program to participants in 23 Northern California counties.
Skilled Nursing and Rehabilitation / Home Care
We offer many skilled services right from the comfort of your home. This includes Injectable drug ‘Teach and Train’ support for clients and family members by a Registered Nurse, Case Management of chronic diseases, specialized respiratory nursing care for patients with tracheostomies, ventilators, or CPAP machines, and Medical Social Workers for referrals to community resources. Our Rehabilitation Services include Physical, Occupational, and Speech Therapy.
Home Care
Our Personal Care Attendants and Homemakers offer assistance with hands-on care and daily household duties for short or long-term needs. Whether you are looking for someone to help care for an aging parent, or are planning for a surgery that will require some downtime, Home & Health Care Management can help.
Care Coordination & Transitional Care
Our agency is leading the way in transitional care here in the North State. We are a Lead Organization for the California Community Transitions (CCT) program and are the first home care agency in the nation to work on the state 8-1-1 project to transition low-income seniors out of skilled nursing facilities back into the community.
Assisted Living Waiver
The goals of the ALW are to Facilitate a safe and timely transition of Medi-Cal-eligible seniors and persons with disabilities from a nursing facility to a community home-like setting in a Residential Care Facility for the Elderly (RCFE), an Adult Residential Care Facility (ARF), or subsidized public housing, utilizing ALW services; and Offer eligible seniors and persons with disabilities, who reside in the community, but are at risk of being institutionalized, the option of utilizing ALW services to develop a program that will safely meet his/her care needs while continuing to reside in a RCF, ARF, or public subsidized housing.
HIV/AIDS Services
We have been providing care to the HIV/AIDS population throughout the North State for more than 25 years. Our case managers, nurses, and social workers are specially trained to provide compassionate and confidential support services. We can also refer clients to programs that offer financial assistance for medications, housing, food, and more.
Health & Safety
We strive to support the public, as well as businesses here in The North State, live safe, healthy, and independent. This is why we provide many health and safety-related services like flu shots, CPR & First Aid classes, and more. We offer these both in our offices, and on-site, to meet you and your organizations’ needs.
